Position Overview:

The Licensed Clinical Social Worker provides clinical expertise, leadership and oversight in a variety of ways as a member of the Enhanced Care Management team. The ECM team provides care management to POLM program participants who are IEHP members. With whole person care principles employed, the integrated care team works with IEHP members and their families, as well as health care and community partners, to coordinate seamless access to health services. This care management is broad, considering that the experience of homelessness is accompanied by urgent health liabilities. Care management can include engaging and assisting with IEHP membership, comprehensive health assessment, linkage to care partners and health organizations, appropriate behavioral health interventions, facilitating care transitions, health education and whole person care education. The LCSW is the lead behavioral health specialist on the team.

The LCSW accomplishes their role through a respectful, compassionate and hopeful style, guided by the company’s values of integrity, excellence, dignity and accountability.


Essential Functions:

  • Carries a case load of IEHP Members with primarily behavioral health needs who are also enrolled in a POLM program.

  • Utilizes brief behavioral health therapeutic interventions with the goal of improving the IEHP Member’s ability to engage and manage their own health choices.

  • Care coordination appropriate for those assigned to case load could include but not limited to:

  • a. Conduct a whole person assessment as directed by RN and appropriate to the scope of clinician

  • b. Develop a member care plan and review/revise/update as needed

  • c. Communicate care plan to member, approved family and all care partners

  • d. Coordinate with health partners to support the members needs, including care transition plans

  • e. Document care plan interactions and generate monthly reports

  • f. Provide crisis intervention to individuals, as well as providing support and clinical guidance to others who engage in this work

  • Provide training on behavioral health conditions and evidence-based treatment to Enhanced Care Management Team

  • Provide consultation to non-licensed ECM team members regarding tasks of a clinical nature

  • Provide informal training and coaching on behavioral health conditions to Emergency Shelter Staff, upon request

  • Coordinate with POLM team and external partners to support members care plan

  • Participate in care team meetings to share information

  • Participate in mandatory partner meetings, or internal POLM meetings.

  • Ensures documentation is accurate and in compliance with regulatory requirements and accreditation standards, including IEHP requirements

  • Embodies the highest ethic with members, including confidentiality, serving all persons in honoring ways, awareness of trauma informed care for those experiencing homelessness


Education and Qualifications:

  • Master’s degree in Social Work from an accredited institution

  • Possession of an active, unrestricted and unencumbered Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Health Sciences

  • Minimum five years of experience performing or facilitating Behavioral Health services

  • Minimum one year care management experience

  • Bilingual preferred

  • Excellent relational skills

  • Excellent communication skills

  • Excellent problem solving and conflict resolution skills

  • Thrives in a collaborative environment

  • Ability to work with a diverse population of people experiencing homelessness


Requirements include:

  • Current CA Driver’s License, proof of Automobile insurance and clean driving record

  • Active LCSW license


Physical & Mental Demands:

  • Ability to multi-task and stay focused with many interruptions

  • Ability to stay composed in difficult situations

  • Working at desk for extended periods of time

  • Lifting and carrying up to 25 lbs. for short distances
