Position Overview:
The Care Manager works to connect individuals to services and resources as part of our Enhanced Care Management team (ECM). The ECM team provides care management to POLM program participants who are also IEHP members. This service is a Medi-Cal benefit offered to those with complex needs. With whole person care principles employed, the integrated care team works with IEHP members and their families, as well as health care and community partners, to coordinate seamless access to health services. Care management can include engaging and assisting with IEHP membership, developing a care plan under the direction of the RN Supervisor, and working with care partners and health organizations in the community. The Care Manager supports the overall work of a four-person ECM team.
Essential Functions:
Carries a case load of IEHP Members as directed by the RN Supervisor.
Care coordination appropriate for those assigned to case load could include but not limited to:
a. Conduct a whole person assessment as directed by RN and appropriate to the scope of clinician
b. Develop a member care plan as directed by the RN Supervisor
c. Communicate care plan to member, approved family and all care partners
d. Coordinate with health partners to support the members needs, including care transition plans
e. Document care plan updates
Coordinate with POLM team and external partners to support members care plan
Participate in care team meetings to share information
Ensures documentation is accurate and in compliance with regulatory requirements and accreditation standards, including IEHP requirements
Maintain regular contact with program participants
Advocate for the needs and rights of the IEHP member within healthcare and social service setting.
Embodies the highest ethic with members, including confidentiality, serving all persons in honoring ways, awareness of trauma informed care for those experiencing homelessness
Education and Qualifications:
Two (2) years in a service setting, health or social services preferred
Experience preferably in an HMO or Managed Care Setting
Successful experience working in a team environment
Excellent relational skills
Excellent communication skills
Excellent data entry skills
Experience with electronic health documentation preferred
Ability to work with a diverse population of people experiencing homelessness
Bilingual (English/Spanish) preferred
Bachelor of Science (BS) in Health Care Management or related field
Requirements include:
Current CA Driver’s License, proof of Automobile insurance and clean driving record
Physical & Mental Demands:
Ability to multi-task and stay focused with many interruptions
Ability to stay composed in difficult situations
Working at desk for extended periods of time
Lifting and carrying up to 25 lbs. for short distances
